HISD Student Continuum of Care
HISD Student Continuum of Care
HISD’s continuum of care model reimagines how we serve and support all students. We recognize that every student deserves a best-in-class education, and providing comprehensive support will help them to become active members of the community, better preparing them for 2035 and beyond.
What We do
- Identify and assess the needs of students and families to connect them with appropriate resources, services, and programs.
- Partner with Sunrise Centers to refer all unmet needs of students and families.
- Monitor and track the effectiveness of interventions and services provided to students and families to reduce chronic absenteeism and prevent dropouts.
